Application for Graduation

Please fill out the form below to apply to graduate in May 2026. Carefully fill out the fields below. You are also required to pay the $150 graduation fee.

Student Number and Legal Name

The name that will appear on your diploma is the name on record in MOSAIC. Please review your “Primary” name as it appears on MOSAIC in Student Services Center > General Info > Names > Primary. If you require a name change, please notify Student Records (divreg@mcmaster.ca) with your request. You must include legal documentation to substantiate your request. The following legal documents will be accepted: name change certificate issued by a province, birth, marriage or naturalization certificate, current passport, current driver’s license, or court order.
